The tirzepatide dose & cost ladder
Two things this page does: shows the FDA-label titration schedule for the brand products, and computes what dose-laddered pricing does to your year. One thing it will never do: calculate injection volumes. Compounded concentrations vary by pharmacy, drawing errors are a documented FDA concern, and that math belongs to your pharmacy label and prescriber — full stop.
| Label schedule (brand) | Dose |
|---|---|
| Weeks 1–4 | 2.5 mg/wk |
| Weeks 5–8 | 5 mg/wk |
| Then, as tolerated, +2.5 mg every 4+ weeks | 7.5 → 10 → 12.5 → 15 mg/wk |
10, 12.5, or 15 mg weekly are the label maintenance doses; your prescriber sets your actual schedule. Compounded programs may use non-label schedules — that is part of the "personalized dosing" gray zone.
Ladder vs flat, over 12 months
Both assume calendar-month billing; 4-week billers add ~8.7% — restate first with the true-month tool.
Label schedule per the brand prescribing information, shown for cost planning and expectation-setting only. This is not dosing advice; never adjust dose or calculate volumes yourself.
