Tirzepatide in California (2026): every provider, the real price
36 priced telehealth programs serving California, each with its billing basis, its source, and the date it was checked — because a “$99/month” billed yearly up front and a “$199/month” billed every 28 days are not the same price, and most roundups pretend they are.

Quick answer
Compounded tirzepatide is a state-licensed 503A pharmacy preparation of tirzepatide, a dual GLP-1/GIP receptor agonist. It is not FDA-approved and not therapeutically equivalent to Zepbound® or Mounjaro®. In California, published prices reported across trackers in August 2026 run about $97–$399 per month, with most standing programs between $149 and $299; billing bases differ (4-week cycles, annual prepay, intro promos), so restate before comparing. The lowest first-party-verified rate on this page is $139/month (NexLife, 12-month plan; $169 month-to-month, same price at every dose, $0 membership — our featured partner, disclosed). Since the shortage resolutions, compounding proceeds only under the 503A patient-specific pathway with a documented clinical need; Medi-Cal stopped covering GLP-1s for weight loss on January 1, 2026. Individual results vary; talk to a licensed clinician.

NexLife in California: flat at every dose, billed by the calendar
Most of the spread in the table below comes from structure, not medicine: 4-week billing adds a 13th charge each year, memberships stack beside the medication, and dose ladders raise the price mid-treatment. NexLife’s program removes all three, which is why it sits in the top five of this page on the math alone.
- Calendar-month billing — 12 charges a year, not 13
- One price at every dose, 2.5 → 15 mg, locked while enrolled
- $0 membership; provider consults, nutrition guidance and support included
- Cancel with 30 days’ notice; term plans optional, never required
Microdose tirzepatide from $129/mo · semaglutide from $119/mo · ODT formats priced separately (no published human PK data for that route — our evidence review). First-party verified 2026-08-24.

How to read this table like a buyer
Four structures move the real price more than the sticker does
The billing calendar. A program billing every 4 weeks charges 13.04 times a year. A “$249/month” on that cycle is really $270.62 per true month — $3,247 a year, not $2,988. That single trick is why this site restates everything to a 30.44-day month; the calculator does it for any quote in five seconds.
Membership stacking. The cleanest example in this table: a flat $199 medication fee with a roughly $79 membership billed beside it, for $278 all-in. Neither number is dishonest on its own; comparing the $199 against another program’s all-in figure is.
The dose ladder. Several programs price 2.5–5 mg aggressively and step up sharply at 7.5 mg and above, while flat-at-every-dose programs (NexLife, bmiMD, HealthRX, Willow, Mochi’s medication fee) cost the same whether you titrate or not. If you expect a long titration, the flat structures usually win by month six even from a higher start.
The savings theater. Roundups love “save $937/month vs Zepbound,” computed against the ~$1,086 list price. Almost nobody paying cash pays list: Lilly sells Zepbound single-dose vials direct to self-pay patients for roughly $299–$449/month depending on dose [verify current LillyDirect pricing]. That is the honest brand benchmark — and against it, compounded programs save real money, just hundreds rather than the advertised near-thousand.
California coverage & rules · 2026
What changed on January 1 — and what most roundups still have wrong
Through 2025, Medi-Cal covered GLP-1 medications for weight loss with prior authorization, and several comparison sites still say it does. California’s 2025 Budget Act eliminated that coverage for weight-loss indications effective January 1, 2026; coverage for qualifying type 2 diabetes (Mounjaro, Ozempic under their diabetes indications) continues with prior authorization. Verify with Medi-Cal before relying on this — but do not rely on pages last updated before the budget passed.
Compounded tirzepatide itself is cash-pay: not covered by Medi-Cal, Medicare, or most commercial plans. Brand-name Zepbound is generally FSA/HSA-eligible with a prescription; some administrators accept compounded GLP-1s with a Letter of Medical Necessity — ask yours, in writing.
Is it legal here in 2026?
The FDA declared the tirzepatide shortage resolved in December 2024 and the semaglutide shortage resolved February 21, 2025, ending the shortage-era pathway for broad compounding; 503B outsourcing facilities had to wind down tirzepatide by March 19, 2025. What remains is the 503A patient-specific pathway: a state-licensed pharmacy compounding for a named patient whose prescriber documents a clinical reason the approved products won’t serve them. “Cheaper” is not a documented clinical reason, “personalized dosing” is the contested gray zone, and Eli Lilly has sued telehealth sellers it says cross the line. California layers some of the country’s strictest board-of-pharmacy and telehealth oversight on top — a legitimate program can name its dispensing pharmacies and its prescribing medical group without being asked twice.
